Dear Cardholder Name,

This letter contains important information regarding the replacement of your <<FI NAME>> &optvar1 card.

<<FI NAME>> was recently notified that your <<debit/credit>> card information may be at risk for possible fraud. It may have been compromised at a third party location. Since your security is our top priority, we have reissued you a new card. This account will have the same account number, however your card will have a new expiration date and three digit security code on the back. This will reduce the chances of your card being used for unauthorized transactions.

Once you receive your new <FI name>> <<debit/credit>> card, follow these steps to begin using your new card:

  1. Activate your new card by calling the number displayed on the front of the card.
  2. Destroy your old card.
  3. Contact any merchants who you have set up for automatic bill pay as soon as possible to avoid any disruptions. (Examples: internet service providers, telecommunications providers, health clubs, etc.).

You can also take steps to protect your account against fraud:

  1. Review your statements on a monthly basis.  If you see any suspicious transactions, report them to <<FI NAME>> immediately.
  2. The Fair Credit Reporting Act may allow you to place a fraud alert on your credit report. The three main credit reporting agencies are Equifax, Experian and Trans Union.
  3. Please notify us when traveling.

<<FI NAME>> and <<Visa/Mastercard>> are serious about our obligation to protect your account. We apologize for any inconvenience this may have caused.

Sincerely,

Card Services
